Purpose: Provide HIV Health Care providers of the Five
County Region with up to date information and resources related to HIV
Intervention Services & Case Management Programs.
Strategy: Maintain up to date Program Manuals for HIV
Health Care Providers. Providing easy access to information and
program related forms. |

EIS Clinic Services Ryan White Part C Mission:
The Mission of the Mountain Counties EIS Program is to promote high
quality HIV related patient care, support services and HIV education
at the four HIV clinic sites in Lassen, Modoc, Plumas, Sierra and
Siskiyou Counties.
Purpose: To provide health, psychological,
educational and nutritional assessments and counseling to establish
goals for therapy and primary care goals for persons with HIV
Disease or AIDS.
Purpose: To provide a coordinated approach to the highest quality of
care and services for consumers living with HIV disease.

HIV Case Management Programs Mission:
Purpose: To provide appropriate medical treatment to prevent or delay
the progression of the disease that results from the HIV infection and
to coordinate available services, provide information and provide
education to HIV-infected persons to prevent the spread of the HIV
infection to others.
- Bridge Program
Purpose: To prevent further transmission of HIV
within communities of color that are disproportionately impacted by
HIV by increasing the number of HIV-infected individuals who are
successfully engaged in comprehensive HIV care, treatment, and
prevention services. CSP - Ryan White Part B CARE Services Program
Purpose: To provide for the care and assistance of HIV+ persons and
their families.
Purpose: To utilize an interdisciplinary team approach to case
management, each client being assigned both a nurse case manager and a
social work case manager. To access needs for persons with HIV
Disease or AIDS and to promote access to high quality health care.
Purpose: To help address the housing and supportive service needs of
homeless or imminently homeless persons living with HIV Disease or AIDS
and their families.
